To investigate the relationship between pubertal growth spurt in body height, skeletal
maturity stage of the Hand-wrist, tooth calcification stages and maxilla and mandibular
growth in normal occlusion, the author tooth the Hand-Wrist radiograph, lateral
cephalogram and panoramic view from 464 subjects(boy 220, girl 244), aged 7 through
17 years, having normal occlusion. Skeletal maturity was assessed on the hand-wrist
view by Fishman's SMI method and jaw growth was determined by 3 mandibular linear
measurements : Ar-4, Ar-Pog, Go-Me in the cephalograph. Tooth calcification stage
was rated on the panoramic view using the method described by Demirjian in which 8
stages of calcification. Menarche age was investigated and pubertal growth spurt was
assessed with longitudinal data of body height from 7 to 17 years. The results were as
follows :
1. The peak height velocity(PHV) during pubertal growth occurred at the age of 12Y
l1M boys, l1Y 4M in girls and 17 months earlier in girls than in boys. The period of
ONSET to END of the pubertal growth was 5Y 1M in boys, 4Y 5M in girls and longer
in boys, 8.7cm/year in girls and greater in boys than in girls.
2. In the skeletal maturity stages of hand-wrist, the pubertal growth spurt appeared
between SMI level 4 and 5 in boys and SMI level 6 in girls. The ossification of the
ulnar sesamoid bone(SMI level 4) occurred 7 mouths earlier than PHV in boys and
about 14 mouths earlier than PHV in girls.
3. PHV was coincident with G-stage of maxillary and mandibular canine, mandibular
first and second premolar, mandibular second molar in boys, and G-stage of maxillary
and mandibular canine and F-stage of mandibular second premolar and mandibular
second molar in girls.
4. The mean age of menarche was 12Y l1M and the menarche occurred at 19 months
after maximum pubertal growth.
5. The correlation coefficients(r) were 0.73 between PHV and skeletal age, 0.72-0.75
between PHV and calcification of the teeth, and 0.82-0.84 between skeletal age and
calcification of the teeth. The correlation coefficients were 0.48 between menarche and
PHV, and 0.61 between menarche and skeletal age.
6. Peak velocity in maxilla and mandibular growth was coincident with PHV during
pubertal growth curve in both boys and girls. Particularly, Ar-Pog growth curve in
dicating mandibular growth was coincident with pubertal growth spurt in boys height.
The strong correlation between PHV, skeletal age, calcification of the selected teeth and
monarch suggest that these indicators might be used efficiently in estimating the level
of skeletal development and planning the orthodontic treatment.
